✦ Synopsis

We have measured increment thresholds for a 4.1 cycfdeg grating in 24 de-novo patients (Hoehn and Yahr stage I to 111) with idiopathic (IPS) and 9 patients with nonidiopathic parkinson syndrome (N-IPS) and for comparison 26 age-matched normal controls.

Patients with positive L-Dopa and apomorphine response were tentatively classified as IPS, patients not responding as N-IPS. The latter encompassed 4 patients with vascular lesions in CT/MR imaging and 5 with suspected multisystem atrophy.

8/24 patients with IPS presented with unequivocal threshold enhancement. 4/24 patients with IPS and 219 patients with N-IPS showed slightly enhanced thresholds. All other patients were normal. We conclude that in the diagnosis of early IPS enhanced increment thresholds contribute in 1j3 of the IPS patients to the differentiation from nonidiopathic Parkinson syndrome.
