We agree with Zesiewicz et al.'s findings and emphasize that simply increasing GABAergic stimulation does not necessarily result in tremor improvement in ET. It is clear that more needs to be learnt on the pharmacologic basis of ET.

To clarify whether primidone itself, and not only its metabolite phenobarbitone, suppresses essential tremor, the effect of a high single dose of primidone was tested. Of 11 patients, 8 showed a reduction of their tremor by 54%-69% for up to 28 h. The serum concentration of primidone was as expected
Twenty patients with tongue tremor associated with essential tremor are reported. Patients were unaware of the tongue tremor, and voice disturbance was a complaint in only one patient. Three patients had an isolated tongue tremor. Hand tremor was present in 16 patients. Dystonia, myoclonus, 4 m 25lM

The study by Djaldetti and colleagues 1 describes eight cases of camptocormia and raises the question of whether this syndrome is a form of dystonia. The following case illustrates that camptocormia may be a form of dystonia in Parkinson's disease (PD). ## Case Report This 75-year-old man develop
