Letter to the Editor: Response to Drs. Westerink and Alberts

During the course of this study, the final target volume remained constant, specifically the ipsilateral flank and entire width of the vertebral body. The field size variations appeared to be related to the median age of the child at irradiation, (i.e., the older the child, the larger the field). Therefore, the "symmetry" of all the fields was unchanged with orthovoltage or megavoltage. This relation may have been preserved by the small number of radiation oncologists treating children during this study.

As we stated in our article, the discrepancy in late orthopedic effects may be due to unfavorable depth dose characteristics associated with orthovoltage. However, it does not appear to be due to different anatomic volumes of irradiation between the two groups.
