Background and Objectives: The 1,450 nm diode laser has been found to be effective for the treatment of acne lesions on the back. This laser causes selective thermal damage and dysfunction to sebaceous glands. To evaluate the efficacy and safety of the 1,450 nm diode laser in the treatment of inflammatory facial acne vulgaris. Study Design/Materials and Methods: Nineteen patients with inflammatory facial acne were treated with the 1,450 nm diode laser at 4-6 week intervals. Based on lesion counts, patients were stratified as severe ( >50 lesions), moderate (15-50 lesions), or mild (<15 lesions). There was no control group. Results: All patients had reductions in acne lesions. Lesion counts decreased 37% after one (P < 0.01), 58% after two (P < 0.01), and 83% after three (P < 0.01) treatments, respectively. When stratified by severity, significant decreases in acne lesions were found for groups with moderate and severe acne. Decreases in lesions counts occurred in the mild acne group but was not statistically significant. Treatment-related pain was well tolerated, and adverse effects were limited to transient erythema and edema. Conclusions: This study documents the safety and efficacy of the 1,450 nm diode laser for the treatment of inflammatory facial acne. Clinical improvement was seen in all patients even in those refractory to previous treatment with oral isotretinoin. Topical anesthetics should be used to minimize pain associated with treatment.
