Abstract

There are many different surgical approaches to the pituitary gland. The earliest approach was intracranial. Extracranial methods (transsphenoidal) then became briefly popular but were subsequently dropped in favor of a return to intracranial methods. Now extracranial approaches have definitely returned to favor, and the reasons for this interesting evolution are discussed. The various techniques are also described. The authors prefer the transseptal‐transsphenoidal variation of the extracranial approach and explain the advantages.
