✦ Synopsis


A 60-yr-old healthy white man is seen with a squamous cell carcinoma of the larynx. Biopsy and monary function tests reveal a normal forced expiratory volume, vital capacity, and flow-volume curve. mapping of the lesion were performed at the time of direct laryngoscopy, revealing an ulcerative lesion of the right vocal cord and ventricle, with apparent extension to the mucosa over the arytenoid. The lesion extends to the anterior commissure and involves 2 mm of the opposite true cord. The cord's mobility is impaired, but it is not fixed. The lesion extends 8 mm below the free edge of the true cord in the middle one third of the glottis. The false cord is slightly edematous, as is the mucosa of the medial wall of the pyriform sinus. The aryepiglottic fold and epiglottis are normal. No nodes are palpated on either side of the neck.

The patient is a bank executive and relates a 40-pacWyear smoking history and that he drinks two martinis for lunch daily. Chest x-ray films, EKG, CBC, serum electrolytes, prothrombin time, and partial thromboplastin time are normal. Pul-l. What further diagnostic tests or evaluations, if any, would you order?
