T he article by Orozco et al 1 in this issue reports an extensive experience with 221 patients undergoing selective decompression of varices for bleeding. This article illustrates several important points but also makes us pause and question the role of selective shunts in the overall treatment of patients with variceal bleeding in the 1990s.
