We present a case of paraganglioma of the thyroid. The preoperative evaluation, surgical management, and histopathology are described. Four similar cases that have been reported are summarized, and a hypothesis for the origin of a paraganglioma within the thyroid capsule is proposed. Paraganglia are derived from neural crest tissue and are dispersed throughout the head, neck, thorax, and abdomen. In 1880, Scudder recognized a neoplasm of the paraganglionic tissue associated with the carotid body.' Most paragangliomas are found in association with the carotid bifurcation, the vagus nerve, and the temporal bone. However, they have also been reported to occur in ar- eas where paraganglia are not usually located, such as the orbit, nasopharynx, trachea, mandible, larynx, heart, lung, stomach, duodenum, gall bladder, retroperitoneum, kidney, urinary bladder, uterus, cauda equina, and Haegert et al. first described a paraganglioma
