โฆ Synopsis
A 45-year-old woman presented with pancytopenia, hypoxemia, and an elevated lactate dehydrogenase level. An attempt to aspirate the bone marrow was unsuccessful, yielding only a ''dry'' tap. The trephine core biopsy specimen was hypercellular, showing extensive fibrosis along with a marked increased in reticulin fibers but with relatively few immature cells. Immunohistochemical staining of this sample was positive for CD117, CD79a, and CD10 and negative for CD20, CD3, TdT, CD34, pan-keratin, and HMB-45.
