rhage. 4 Because of the infrequency of such cases, very little is known about neurological consequences of unilateral nigral damage in humans. In the 3 of the 4 cases as well as the present case, hemiparkinsonism was found in the contralateral side to the nigral lesion. [2][3][4] Bradykinesia of the extremities contralateral to the nigral infarction was the most prominent symptom, although tremor was also found. Among the past reported cases, 3 cases showed resting tremor 2-4 and 1 case showed fine tremor accentuated by voluntary movement. 1 Our patient's tremor resembled this latter description. It is also noteworthy that experimental nigral lesions in rhesus monkeys may not necessarily cause tremor, even though they always evoke bradykinesia 5 and that 30% of patients with Parkinson's disease manifest initial symptoms other than tremor. 6 The present case provides clinical evidence that a circumscribed nigral lesion in the human may not cause prominent resting tremor such as pill-rolling tremor.
