✦ Synopsis

Part 1 describes the case of a 74-year-old woman who experienced a worsening of her psychiatric condition shortly after commencing fluoxetine and was found to have a low serum sodium level. The clinical features of hyponatraemia and its causes are reviewed. Part 2 reviews the reports of hyponatraemia and the selective serotonin reuptake inhibitors (SSRIs) and considers the variation in reports between the different SSRls. Measurement of a patient's electrolytes is recommended if there is a rapid decline in mental state having started on an SSRI. KEY woms-Selective serotonin reuptake inhibitors, hyponatraemia, antidiuretic hormone.

There is considerable evidence linking developmental pathology with serotonergic abnormalities. Individuals with mental retardation may be particularly susceptible to dysfunction in the serotonin system. The development of more selective drugs that modify serotonergic function appears to have provid

## Abstract Hyponatraemia (serum sodium arbitrarily defined as less than 135 mmol/L) is an increasingly recognised adverse effect of selective serotonin re‐uptake inhibitors (SSRIs). Its precise prevalence and incidence in the elderly are hard to determine because of confounding factors including o
