โฆ Synopsis
Barshes et al. have provided a detailed and informative review of the role of liver transplantation in an infrequent condition, end-stage liver failure due to graft-versus-host disease (GVHD) after bone marrow transplantation. The authors reviewed cases collected from their own experience, English-speaking medical literature, and the United Network for Organ Sharing liver transplant registry. The one-and five-year patient survival rates were 72% and 63%, respectively. No deaths or liver failures were attributed to hepatic GVHD recurrence. Their review points out that some of the patients had cutaneous GVHD and others had intestinal GVHD. Those with cutaneous GVHD had an acceptable outcome. The authors concluded that concurrent cutaneous manifestation should not be a contraindication for liver transplantation; however, for concurrent intestinal GVHD more data are needed.
