Immunosuppression and Donor Age With Respect to Severity of HCV Recurrence After Liver Transplantation

Older donors have been reported to be associated with increased late mortality and increased fibrosis among patients transplanted for hepatitis C virus (HCV) cirrhosis. On the other hand, this observation has not been observed by every transplant program. Other factors described to lead to progressive recurrent HCV disease include immunosuppression, year of transplantation, viral genotype, and levels of viremia prior to transplantation. The Liver Transplant Program from the Royal Free Hospital in London (UK) has studied 193 HCV cirrhotic patients transplanted with cadaveric donors and evaluated donor age and severity of severe fibrosis, among other factors. Surprisingly, they found no influence of donor age in either late mortality following transplantation or development of severe fibrosis. Furthermore, they found that maintenance azathioprine immunosuppression had a beneficial effect on clinical outcome. This publication is accompanied by a lucid
