To our best knowledge, this is the ยฎrst report of a false positive misleading MIBG scan due to an accessory spleen. Our observation gives further evidence that a positive MIBG scan should not be considered necessarily pathognomonic of neuroblastic tumor, and that biopsy is essential in order to conยฎrm the diagnosis.
