istoplasma capsulatum is a well-known fungal pathogen that is present in the soil and endemic to the Ohio and Mississippi River valleys. Exposure to humans comes from inhalation of airborne spores, and infection usually results in a self-limited pulmonary syndrome. 1 Disseminated histoplasmosis is an opportunistic infection observed in individuals with impaired cell-mediated immunity such as lymphoma, lymphocytic leukemia, acquired immunodeficiency syndrome, and immunosuppressive therapy. 2 We report the first case of disseminated histoplasmosis after orthotopic liver transplantation. Although disseminated histoplasmosis is documented after renal transplantation, 3 it has not been previously reported after orthotopic liver transplantation.
