โฆ Synopsis
Abstract
Background: In the lower leg posttraumatic pathology, we are frequently confronted with tissue defects that are difficult to cover by local or distance means, because of either the poor tissue quality or the precarious local vessels. One of the still available methods for solving these cases remains the crossโleg flap. The authors will try to demonstrate the advantages of this method, by using the crossโleg flap based on perforator vessels. Methods: We are presenting two cases for which we practiced a crossโleg perforator flap, based on perforator vessels emerging from the posterior tibial artery. Results: The results were favorable, demonstrated by immobilization in a comfortable position, perfect flap integration, pedicle flap's division at 14โ21 days, early postsurgery ambulation. Conclusions: The crossโleg perforator flap diminishes many of the disadvantages created by the classic crossโleg flap and can be successfully used in cases in which other procedures are prohibited. ยฉ 2007 WileyโLiss, Inc. Anatomy of cutaneous perforators of the posterior tibial artery were studied in 20 limbs of 10 cadavers. The majority of the perforators (n = 74, 61%) were located in the middle two quarters of the leg, at an average of 18.6 cm (s.d. 4.5 cm; range 10.5-26 cm) from the medial malleolus, or around 54%
