The Problem of Acetabular Loosening in Patients with Total Hip Joint Replacement Summary. Recently aseptic loosening of the acetabular part in patients with allo-arthroplasty has also become a numerically increasing problem. Different opinions exist regarding the etiology, they range from mechanical to tissue factors. Favourable personal experience with careful handling of the bony acetabulum during surgery appeares to confirm that the size and type of treatment of the acetabular wound during implantation of the acetabular prosthesis are significant factors for the durability of the allo-arthroplasty. A formula which was compiled indicates and clearly shows how the increase in area of the acetabular wound depends on the increase of the acetabular radius. In addition, the laws of cellular reaction from general pathology should be the reason to limit damage to living tissue to the absolutly necessary extent.
Zusammenfassung. Die aseptische Auslockerung yon Hiiftpfannen bei der
Allo-Arthroplastik stellt inzwischen ein auch zahlenm~iBig wachsendes Problem dar. l~ber die Ursachen herrschen v o n d e r mechanistischen bis zur geweblichen Ursache unterschiedliche Auffassungen. Gute eigene Erfahrungen mit der vorsichtigen Behandlung der knrchernen Pfanne w~ihrend der Operation scheinen zu best/itigen, dab in der Gr6Be und Art der Behandlung der Knochenwunde bei der Pfannenimplantation wesentliche Faktoren fur die Dauerhaftigkeit der Allo-Arthroplastik liegen. Eine aufgestellte Formel l~iBt ablesen und deutlich erkennen, wie der F1/lchenzuwachs der Knochenwunde vonder VergrOBerung des Pfannenradius abh~ingt. Die Gesetze der zellulgren Reaktionen aus der allgemeinen Pathologie sollten dartiber hinaus Veranlassung sein, das lebende Gewebe mSglichst nur in unbedingt notwendigem Umfang zu sch/idigen.
