An infant with oesophageal atresia and a long gap between the oesophageal ends has been treated by continuous aspiration of the upper pouch, gastrostomy, and elongation of the upper pouch by bouginage, followed by thoracotomy and primary oesophageal anastomosis. The problems of management are discussed.
