The author describes a new modification of a previously described procedure. He describes his technique in 32 patients over an 18-year period utilizing a modified z-plasty with a wide exposure, allowing modification of the platysma muscle, submental fat and submental skin with simple closure. This procedure is limited to patients with aesthetically unacceptable residual redundancy following a complete rhytidectomy or who have declined to have this extensive procedure.
