The ability to absorb protein-bound vitamin BI2 was assessed in six elderly psychiatric patients who had low serum levels of the vitamin but normal Schilling tests. Three of the six had abnormal protein-bound vitamin B12 absorption, suggesting that malabsorption may play a part in the genesis of subnormal serum levels although conventional testing may not show it. The therapeutic implications are discussed.
