โฆ Synopsis
This 10-year-old white female seemed entirely normal and in good health until several weeks before admission. Then, progressive anorexia, lethargy, tiredness, and constant drowsiness appeared. Medical assistance was sought, and a left upper quadrant abdominal mass was detected. She therefore was referred to the Oncology Clinic, where she was found to be a wasted, cachectic girl of the stated age. She gave no history of abdominal pain, alteration of bowel habits, nausea, vomiting, or dysphagia. Physical examination was otherwise unremarkable except for the presence of a rock-hard, nodular, nontender ballotable mass in the left upper quadrant. It emerged from beneath the left costal margin and extended 8.5 cm below that landmark. It measured 7.5 cm in transverse diameter, and went beyond the midline. There was no generalized adenopathy ; specifically, there were no supraclavicular lymph nodes palpable on the left. The only other notable finding was thinning of the scalp hair. The laboratory studies included a completely normal blood count (hemoglobin 14.2 g/dl).
