β¦ Synopsis
Total ankle replacement aims to oΒ‘er a successful alternative to arthrodesis, the existing mainstay and gold standard surgical procedure for disabling ankle arthritis. Following initial attempts at ankle replacement in the 1970s, early optimism proved unfounded as the procedure suΒ‘ered from an unacceptable complication rate and poor prosthetic longevity. With recent advances in biomechanics, material sciences and surgical technique, first generation constrained, cemented designs have been replaced by semi-constrained and uncemented prostheses.The initial results of these designs are more encouraging with reports of up to 94% survivalrates at 5 years, with 93% overall patient satisfaction. Additional clinical and radiographic follow-up is stillrequired to ascertain the longer term results. If acceptable, numerous patients stand to benefit from the addition of this surgery to current orthopaedic practice.This review considers the history and evolution of ankle replacement surgery leading to current designs and reviews their results.
