✦ Synopsis

The SAGES Manual of Peri-Operative Care in Minimally Invasive Surgery is SAGES' second clinical manual, following their bestseller The SAGES Manual: Fundamentals of Laparoscopy and GI Endoscopy. This pocket- sized, portable text provides the practicing surgeon and trainee with comprehensive, authoritative coverage of the pre-operative evaluation of laparoscopic patients, intraoperative strategies and minimization of complications, and post-operative care. In addition, the reader will find thorough discussions of the physiologic implications of CO2 pneumoperitoneum on cardiovascular, renal, pulmonary, hepatic, and immunologic function. Written by SAGES leadership and renowned experts in minimally invasive surgery, the text includes:
β€’ guidelines for pre-operative evaluation of both the healthy and the complex laparoscopic patient
β€’ strategies for successful outcomes, including OR set-up, patient positioning, anesthetic considerations, choice of port-placement and exposure method, port-closure method and hernia prevention
β€’ managing trocar-placement related bleeding, prophylaxis against DVT, prevention of intraoperative hypothermia and subcutaneous emphysema
β€’ when to convert to open methods
β€’ physiologic, immunologic and oncologic implications of CO2 pneumoperitoneum, prevention of port -site tumors, minimizing the risk of gas embolism
β€’ post-operative considerations including fluid management, ambulation and early post-operative performance criteria.
β€’ multiple port arrangements for most operations and port placement schemes that take into account the B.M.I. of the patient. Over 100 figures complement the text. This handy reference is a must-have resource for all surgeons currently using minimally invasive techniques as well as for all surgical trainees preparing for laparoscopic cases and studying for the board examination.

**Reviewer:**Jose Greenspon, MD(University of Maryland School of Medicine)
**Description:**This is a pocket manual reference guide to perioperative care of patients undergoing minimally invasive procedures. The handbook contains specific sections on the distinctive situations encountered in laparoscopic surgery, including management of morbidly obese patients as well as CO2 pneumoperitoneum. The book is parsed into sections covering perioperative, intraoperative, and postoperative management and it ends with a section covering the wide array of physiologic phenomena that are unique to laparoscopic surgery.
**Purpose:**The purpose is to provide a quick "locker-room" reference guide for residents and practicing surgeons regarding patient positioning in the operating room and selection of appropriate port placement, as well as important management guidelines to the pre- and post-operative care of patients undergoing minimally invasive procedures.
**Audience:**Residents and practicing surgeons who seek a concise yet detailed guide to basic perioperative management principles of the patient undergoing minimally invasive surgery are the intended audience.
**Features:**The handbook's four main sections cover perioperative care; intraoperative management, port placement and positioning; postoperative management; and physiologic implications of pneumoperitoneum and minimally invasive methods. Within each section chapters usually detail, in outline fashion, general considerations as well as specific workup and management guidelines for each topic. The chapters are typically short yet surprisingly detailed, thus they can be read quickly with high efficiency. In the intraoperative section of the book, every chapter is accompanied by excellent diagrams featuring patient and port positioning options, and where appropriate, the specific considerations one must take with the morbidly obese patient. The final section is dedicated to the physiologic implications of CO2 pneumoperitoneum and laparoscopy, a feature unique to this manual.
**Assessment:**This concise manual accomplishes and surpasses all of its goals. The superb quality and clarity, and frankly, brevity of the chapters, allow a reader to glean a great deal of information in a short period of time. Several chapters not only cover universal positioning techniques, but also include author recommendations and common alternatives one may encounter based on surgeon preference. As a resident in the operative years of my training, I find this guide to be an excellent and unique addition to the growing library of pocket surgical resource manuals.
