Abstract

This article introduces a method of prioritizing medication errors for root cause analysis and corrective action. The tool is used to assess medication errors across two dimensions, one of which is degree of injury to the patient (severity dimension), the other is the place within the system where the deviation first occurred (complexity dimension).
