NANB-PTH could be directly compared in those recipients who received blood which was surrogate markernegative to recipients whose blood had not been screened for the two markers, such a study is unlikely to be undertaken in light of existing information.
Clearly, surrogate screening for NANB carriers is a poorly specific, insensitive, interim measure to serve only until NANB serologic assays become available. Such screening cannot prevent the majority of cases of PTH and is wasteful. The majority of units excluded as a result of screening, 1 to 3% for elevated ALT levels and 4 to 6% for anti-HBc positivity, are noninfectious. To insure an adequate supply of blood now and in the future, recruitment efforts must be intensified to increase the number of volunteer donors. Autologous transfusions for elective surgery and intraoperative blood salvage should be encouraged. This will further reduce the risk of NANB-PTH and also make blood available to meet true needs. Serological assays for NANB, when developed, are unlikely to detect all infectious donors. Even more important is the curtailment of unnecessary transfusions, which is a major problem at the moment. Transmission of hepatitis from unneeded transfusions is a doubly destructive abuse which deprives others of blood and induces disease.
