Intra-arterial Cis-diammine dichloroplatinum (11) administered to five patients produced systemic and sustained hypertension in four of them. This finding persisted for six months despite discontinuation of the drug. This complication has not been observed in patients receiving the drug by the intravenous route. Possible etiologic factors are discussed. Cancer 47945-247, 1981. IS-DIAMMINI: dichloroplatinum I1 (CDDP) (NSC-C 119875) is an inorganic platinum compound with an antineoplastic effect. Toxicologic studies in man, dogs, and monkeys indicate that lethal doses produce chemical and anatomic damage in the gastrointestinal tract, bone marrow, lymph nodes, liver, and kidney.' Human studies demonstrate that a single dose of CDDP exceeding 1.95 mg/kg is likely to produce renal impairment.3 Clinically recorded side-effects include peripheral neuropathy , ototoxicity (tinnitus and highfrequency hearing loss above speech tones), and ophthalmologic disturbances.Y,ll
Intra-arterial chemotherapy is used to deliver high tumoricidal concentrations. In this context, a clinical trial with CDDP in melanoma and other localized tumors was undertaken.13 The range of side-effects was similar to those previously reported for CDDP. This paper records an additional side-effect-systemic hypertension. This has previously not been reported with CDDP or other intra-arterial chemotherapy.4*7,14*15
Materials and Methods
Intra-arterial CDDP was administered to four adolescent patients (12-16 years) with osteosarcoma, and to one patient with carcinoma of the maxillary sinus. The tumors were inoperable, or unresponsive to conventional therapy, and were located in anatomic sites, as depicted in Table 1 .
