Subdiaphragmatic Esophageal Duplication Cyst in a Child

A 5-year-old girl presented with a 5-month history of recurrent epigastric pain. The pain ensued immediately after meals and receded spontaneously. Results of physical examination and laboratory tests were within normal limits. A sonographic examination was performed using an XP 128 ultrasound scanner (Acuson, Mountain View, CA) and a 7-MHz sector transducer and revealed a round, anechoic mass without posterior enhancement. The mass measured 1 cm in diameter and was located in the upper abdomen, between the left lobe of the liver and the cardia. The mass's walls were echogenic and well defined (Figure 1). CT confirmed the presence of the mass and its location.

The patient underwent exploratory laparotomy, and the cystic mass was found to be attached to the right side wall of the intra-abdominal segment of the esophagus. Histologic examination revealed 2 distinct layers of muscular tissue and a lining of ciliated pseudostratified columnar epithelium, characteristic of a duplication cyst. All symptoms subsided postoperatively.
