Abstract
In an eighteen month project within the Portsmouth and SE Hants Health District, facilitators helped structure diabetes care in 27 practices. In total 39 practices were visited. The prevalence of diabetes in the practices providing structured care was 1.56% (2,684 diabetics in a population of 177,100). Delivery of structured care within the District increased fourโfold by the end of the project.
