impetigo, erysipelas, and puerperal fever. Rheumatic fever and other serious infections caused by St. pyogenes had been declined during the 20th century.2p3 However, recent outbreaks of acute rheumatic fever and the newly recognized streptococcal toxic shock-like syndrome may reflect a renewed virulence of this ~r g a n i s m . ~

Recent reports demonstrate the potential of this organism to cause dramatic, fulminant, and life-threatening systemic illness5 This entity is termed necrotizing erysipelas or necrotizing fasciitis, but most recently the term streptococcal gangrene is preferred.

This process is extremely rare in the head and neck region, with only 11 previous reports in the otolaryngologic literature.6 This disease is characterized by a rapidly spreading infection of the superficial and deep fascia with secondary necrosis of the overlying skin. The outcome is influenced by prompt recognition and immediate institution of parenteral antibiotics and surgical intervention. It is therefore of utmost importance that otolaryngologists be alerted to this condition.
