✦ Synopsis


Cancer of the base of the tongue is usually not diagnosed until it is fairly advanced; the growth and extension of the tumor around the primary site often involve adjacent anatomic areas of functional importance, such as the pharynx and larynx. Multiple, bilateral lymph node metastases are usually present and make appropriate treatment planning more difficult. The available treatments, surgery or radiotherapy or a combination of both, offer less than ideal results, both in tumor eradication and possible complications and sequelae. The present "problem case" is an example of cancer of the base of the tongue detected relatively early, for which the consultants offer their treatment plans.

CLINICAL HISTORY

A 64-yr-old, healthy, man is seen with a T2N1 MO squamous cell carcinoma of the base of the tongue. Biopsy and mapping of the lesion were performed at the time of direct laryngoscopy by another head and neck specialist.
