1960, with a 4-day history of abdominal pain which was accompanied by vomiting for 12 hours prior to admission. The pain had come on suddenly whilst at work shovelling coal. The patient, attempting to lift a heavy load, had strained and suddenly felt pain in the right loin. The pain was attributed to a muscular strain and had not been very severe in that he had continued with his work after a short rest. Indeed, the patient had continued at work until the day before admission to hospital. The pain was constant, gradually increasing in severity, and had shifted to the right iliac fossa 24 hours before admission.

ON ExAMINATION.-His temperature was 98.4" F., pulse-rate 80 per minute, and blood-pressure 130/85 mm. Hg. The breath was foetid. The abdomen was obese and showed marked guarding and tenderness with rebound * Now Consultant Surgeon to the Rochdale and District Group of Hospitals.
