β¦ Synopsis
A 40-year-old black man presented with a 2-week history of painful swelling of the left testis. There was no history of trauma. Physical examination revealed a mildly tender swelling of the left testis. He was treated with antibiotics for 3 weeks but there was no significant improvement. He was examined 5 weeks later, on which occasion he still had a swollen left testis which felt hard. At this stage he was referred for ultrasound examination, which showed an enlarged testis measuring 6.0 cm with diffuse areas of decreased echogenicity throughout [Fig. and)]. There was no associated hydrocele. The right testis was normal with a maximum length of 4.0 cm [Fig. and)]. Sonograms of the abdomen showed no adenopathy, ascites, or hydronephrosis. The provisional presurgical and ultrasonographic diagnosis was left testicular tumor.
At surgery a left orchidectomy was performed.
