Dear Sir, in a recently published paper in this journal, Takematsu and Tagami [2] reported elevated soluble interleukin-2 receptor (sIL-2R) levels in suction blister fluids from psoriatic skin. Although there was no interleukin-2 or interferon-gamma detection, local T-cell activation was suggested based on these data.

We would like to critically examine this assumption. The lack of a determination of sIL-2R in the serum of the patients leaves open the possibility that the sIL-2R levels detected are the result of filtered and diluted serum which contains sIL-2R levels between 280 and 500 U/ml (mean, 429; standard deviation, 75, n = 10) in healthy control patients and elevated sIL-2R levels in psoriatic patients [mean, 830; standard deviation 429, n = 23 (our unpublished results); data obtained with a ELISA provided by T Cell Science, Cambridge, USA]. The slight increase of suction blister sIL-2R from involved skin compared with uninvolved skin from the same individual, might be due to an augmented blood flow in inflamed psoriatic areas.
