inadvertent activation of the wrong switch or circuit-breaker. Specific preventive measures were proposed for each type of error. The paper includes a critical discussion of various approaches to error prevention. It is pointed out that ergonomic and technical improvements are the most effective of the proposed measures because they take account of the inherent unreliability of the human operator. Unfortunately, the implementation of such measures tends to be expensive when changes to an existing system are necessary.
