Barney and Gilbert wrote that 'Cancer of the prostate often becomes very extensive and even metastasizes before urinary symptoms call attention to its presence'. Several detailed reviews of causes of ureteric obstruction (MacLean and Fowler, 1956; Kaufman, 1962) and of modes of extension of prostatic carcinoma (Bumpus, 1926; Franks, 1956) have not described a similar example. T h e patient reported here presented with acute renal failure due to bilateral ureteric obstruction involved by an extending prostatic carcinoma, and well illustrated the problems of management.
