โฆ Synopsis
An analysis of 80 patients with seminoma of the testis treated at Brooke General Hospital, from July 1947 to March 1971, is presented. Forty-nine patients with Stage I disease were treated prophylactically to the mediastinum and left supraclavicular area in addition to receiving periaortic and pelvic irradiation. Twenty-two patients with Stage I disease received only periaortic and pelvic irradiation. No recurrences have been observed in Stage I patients with either method of treatment. The crude survival rates for the entire group of 80 patients at 5, 10, and 15 years were 97, 93, and 92%, respectively.
HE IMPORTANCE OF IRRADIATION IN THE
T management of patients with seminoma of the testicle has been recognized for many years. It is generally understood that this form of cancer has the best prognosis of the many different cancers that afflict mankind.
T h e work of Friedman4--" represents a milestone in the development of improved treatment techniques providing better untlerstmding of optimum dosage and normal tissue tolerances. Friedman and Moore' and Dixon and Moore2 classified testicular cancer into clinical groups enabling investigators to attain a more organized approach to treatment and improving the ability to evaluate results.
Maierlz and Kurohara'o each reported a series of patients who received prophylactic mediastinal irradiation. Kurohara9 also has presented a group of patients who received periaortic and pelvic irradiation alone tollowing orchiectomy. Johnson8 and Castrol have both advocated periaortic and pelvic irradiation alone atter orcliiectomy for patients with Stage I seminoma of the testicle.
