In conclusion, the results of our study indicate that not only the burden of systemic inflammation, but also local cardiac inflammation and activation of cardiac microvessels, may play roles in the pathogenesis of cardiac ischemia and heart failure in the setting of CVD. Furthermore, local cardiac inflammation may be more important for CVD in patients with IRD than in patients without IRD, and heart muscle seems to be more sensitive to inflammation than skeletal muscle. Thus, our findings indicate that a local inflammatory process in the heart could contribute to the increased rate of CVD in patients with IRD.
