Form factors involving the 1 s, 2s, 3s, and 4s states of hydrogen-like atoms have been calculated in the Born approximation. Coefficients of exact analytic expressions in terms of the momentum transfer are tabulated for all transitions to discrete states with n' ~ 10 and l' ~< 3.
