Abstract

BACKGROUND

Immunosuppressive agents may have an impact on ischemia/reperfusion injury. Both renal warm ischemia (RWI) and cyclosporine may contribute to chronic allograft dysfunction and delayed graft function (DGF). Interaction between cyclosporine and RWI in generating renal injury is a controversial area.

METHODS

A search of the Medline database and OVID was done to identify articles related to the effects of cyclosporine on RWI.

RESULTS

Both animal and human studies demonstrated detrimental effects of cyclosporine on injury and fibrosis in the kidney subjected to a period of RWI.

CONCLUSIONS

Cyclosporine can sensitize the kidney to fibrosis induced by RWI. We recommend alternative approaches in immunosuppression including reduction in cyclosporine or cyclosporine‐free protocols when RWI is expected, as in non‐heart‐beating donors or in hemodynamically unstable donors.
