Patients in group i followed for ten years lost a mean of 0-84 SD in height. For an adult male, this represents 5-6 cm and for a female 5 cm. A decision to institute GH treatment must take into consideration the final height prediction and hence the likely benefit of long-term intensive and expensive therapy. The GH requirements for treated children with ALL will vary between centres. In centres where the prevalence and severity of growth retardation are considerable, many children should be offered GH treatment. We consider that the mean loss of height over ten years in our children is not sufficient to justify long-term GH therapy in the majority. The clinical dilemma is then to identify the few who should receive treatment. We suggest that those children who are below the tenth centile or whose growth rate is persistently poor after completion of chemotherapy, where there is GH deficiency, should receive a therapeutic trial of GH.
