When Sir Harold Ridley implanted the first intraocular lens in 1949, he irrevocably coupled lens extraction with refractive surgery. The purpose of the implant remains essentially refractive to this day. However, we are now witnessing an unprecedented flowering of technology for pseudophakia.
Elimination of defocus and astigmatism, implantation of accommodative intraocular lenses and pseudophakic correction of higher-order aberrations require systematic preoperative evaluation and meticulous surgical technique. Surgeons must meet the challenges of understanding and implementing this new technology if they are to ride the coming demographic tidal wave of baby boomers desiring excellent vision without relying on spectacles.
