In this issue of Catheterization and Cardiovascular Diagnosis, Bahl et al. describe their experience using a 0.014 inch hydrophilic guidewire for crossing chronic total occlusions and the results of subsequent intervention. The success rate of 25 of 30 cases (83%) is higher than that traditionally assigned to this strategy, reflecting no doubt in part the ability of the operators.
