THE FOUNDATION MOVEMEKT

The Foundation for Medical Care (FMC) can be defined as an "individual practice system" (I1) for the coordination of services from individual health practitioners with the resources of hospitals and other health care facilities. There is thus an organized activity or federation of fee for service providers into a system that is both community oriented and comprehensive. All professional members are under contract t o render their services in prescribed manners and a t stated fees. Costs and quality are maintained by close utilization (peer) review while capitation limits (risk sharing) are assumed by practitioners in some plans. To insure that coverage will be comprehensive, minimum standards are established and a foundation will not handle policies that do not meet its coverage requirements(3. l * ) .

Pioneered in San Joaquin County, California, in 1954, the F M C concept has spread to over 30 counties (17 foundations) in the state and to a rapidly growing number of areas in other states, including Arizona,
