No capsules were suitable for vegetarians, although the manufacturers of Duloxetine stated in vitro testing had demonstrated that the duloxetine enteric-coated pellets contained within the capsules are stable at room temperature for up to 2 h when mixed with either the apple sauce or juice, which quantitatively allows delivery of the full capsule dose. It is unknown whether this technique can be used with other capsule preparations. Although all were asked, few companies confirmed whether or not their products' excipients were likely to change in the future. It is therefore sensible to periodically check if a drug contains a potentially meat derived excipient using the electronic medicines compendium (www.emc.medicines.org.uk).
This case demonstrated the importance of inquiry into dietary choices that may impact upon a patient's decision to take medication. Although ample preparations are suitable for vegetarians, more restrictive dietary choices could limit prescribing options. Switching to a different brand of drug, or administering the same medication in a different form may solve the problem. In psychiatry, it is also sometimes necessary to enforce medication, raising additional dilemmas.
