## Abstract An additional case of the stiff‐man syndrome manifested by extremely painful, fluctuating muscular spasm and rigidity is reported. Eighteen cases previously reported in the literature are reviewed, and the accumulated indications for a metabolic defect and/or myopathy are presented.
