Two patients with pulmonary metastases from synovial sarcoma and germ-cell tumor developed first bilateral, then repeated unilateral pneumothoraces during combination chemotherapy. It is suggested that chemotherapy may occasionally cause pneumothorax.

NEUMOTHORAX is a rare complication of pul-P monary malignant disease.x This report describes two patients who both developed bilateral pneumothorax during combination chemotherapy.
Patients and Methods
Both patients had pulmonary metastases and were treated with a four-drug s ~h e d u l e : ~ Vincristine 1.5 mg/M2 (maximally 2 mg) was given Day 1 and repeated Days 8 and 15 during the first course. Adriamycin and cyclophosphamide, 50 and 500 mg/M2, respectively, were given together with 0.3 mg/M' (maximally 0.5 mg) actinomycin-D on Day 2. The dose of actinomycin-D was repeated on Days 3 and 4. All drugs were given by rapid intravenous infusion. The course was repeated Day 21 if leukocyte and platelet counts were above 3 x 10Y/liter and 100 x IO9/Iiter, respectively.
