From 1987From to 1990, 45 , 45
children with cerebral palsy underwent percutaneous elongation of the Achilles tendon (71 ankles); the mean age was 5.7 years. Most suffered from spastic diplegia or hemiplegia. A three-step cut technique was used, the limb immobilised in a long leg fibreglass cast and immediate weightbearing allowed. Follow up was from 2 to 5 years. Satisfactory results were obtained with significant improvement in walking in 89%.
R6sum6. L'dquin par rdtraction du tendon d'Achille est fMquent chez les enfants spastiques. Le traitement chirurgical peut am6liorer la fonction des patients ambulatoires. De 1987 it 1990, 71 allongements per-cutan& du tendon d'Achille ont 6td effec-tu~s chez 45 enfants. La majoritd pour des hdmipl6gies ou des dipldgies spastiques. L'~ge moyen au moment de l'intervention 6tait de 5 ans 7 mois, avec des extrOmes de 2 et 9 ans. L'intervention a dtd r6alis~e en per-cutan6, de fafon it ce que la cheville soit amen6e en position neutre. Le membre infdrieur a ensuite 6t~ immobilisd dans un pl8tre cruro-pddieux, le pied it angle droit, le genou en extension. La marche avec appui complet a dt6 permise au 2dme ou 36me jour post-opdratoire. Le plgttre a dt6 trans-form6 en botte platrde it la 2dme semaine et supprim~ g~ la 4dme semaine. Avec un recul de 2 it 5 ans, une amelioration significative de la marche a 6td constatde dans 89% des cas. ll n'y a eu aucune rupture itdrative du tendon d'Achille ni de problkme de eicatrisation.
