โœฆ Synopsis


A diagnosis of perforation of a peptic ulcer at the gastrojejunal anastomosis was made, and the patient was operated on at 9.30 p.m.

AT OPERATION.-under light general anresthesia with curare, the abdomen was opened through a right paramedian incision. Multiple adhesions in the epigastric region made the gastrojejunal anastomosis difficult to visualize, but when this was done a perforated ulcer was located on the posterior part of it. This was closed by a purse-string suture and an omental graft. On further exploration a lesion was noticed on one of the loops of small bowel, which when inspected proved to be a perforated ulcer of the jejunum 12-15 in. distal to the anastomosis and on the antimesenteric border. This was repaired with Lembert's sutures. Drainage was established in the rectovesical pouch and pouch of Morrison, and the abdominal incision closed.

He was dismissed home on May 15, symptom-free, and had remained so one year after operation as long as he kept to a bland diet.

The patient ma-le an uneventful recovery.
