Kano and associates [I) reported a patient with positive human T-lymphotrophic virus type I (HTLV-I) antibodies whose symptoms, similar to those of Shy-Drager syndrome, improved with corticosteroid therapy. They suggested that HTLV-I infection is related to Shy-Drager syndromelike neurological disorders. In fact, tremor has been reported in 27% and mild cerebellar ataxia in 5% of patients with usual HTLV-I-associated myelopathy (HAM), in addition to cardinal features of HAM; i.e., slowly progressive spastic paraparesis and mild sphincter and sensory disturbances 123.
