Therapeutic successes with retinoids include the treatment of acute promyelocytic leukaemia and the reduction of secondary primary head and neck cancers. There have also been some unexpected negative clinical results, particularly in the field of chemoprevention. New discoveries in molecular biology have provided insights into these diverse effects, and the search continues for more active retinoids as well as ways to enhance the activity of earlier ones. This report briefly reviews methodological issues that arise when promising laboratory or epidemiological findings concerning retinoids are tested in a clinical setting.
